How do I sign up for McFarland clinic?
Visit the MyChart website and click sign-up, or click the button below. Complete the online form and verify your identity. Download the free MyChart app for your mobile device. What is MyChart?
MyChart offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With MyChart, you can use the Internet to:
How do I access my health information in MyChart?
The login name and password you choose will be used to access your health information in MyChart and are unique codes that identify you in the MyChart system. Any inquiries and entries you make in MyChart will be logged with your identity and may become part of your official medical record.
How do I use my MyChart ID and password?
MyChart ID and Password – You will be provided with an access code when you initially enroll in MyChart. You must use the access code that is given to you to create a unique personal login name (called “MyChart ID”) and password. Once the access code has been used, it will no longer be valid.
Can I view a family member’s health record in MyChart?
Yes you can. This is called Proxy access and allows a parent (or guardian) to log into their personal MyChart account, and then connect to information regarding their family member. A Proxy Access Form must be completed and submitted. If a patient is not mentally competent to authorize access to their record, please also provide copies of relevant documents (guardianship letter, etc).
Is MyChart secure?
Access to information is controlled through secure access codes, personal ID’s, and passwords. Each person controls their password, and the account can only be accessed with that password. Keeping your password private is the most effective way to keep your health information private.
How do I request a copy of my complete medical record?
In accordance with HIPAA regulations, obtaining copies of your complete personal health records may be facilitated though our Health Information Management department.
What is MyChart activation code?
Patients who wish to participate will be issued a MyChart activation code during their hospital or clinic visit . This code will enable you to login and create your own user ID and password. If you were not issued an activation code, you may ask to sign up during your next office visit or complete and mail in a sign-up form.
