What is mymosaic by MyChart?
myMosaic by MyChart provides you with online access to your medical record. It can help you participate in your health care and communicate with your providers. Review summaries of your previous appointments, including issues addressed during each visit, your vital signs, and tests or referrals that were ordered.
What is MyChart?
Valley Health and Inova Health System have formed an alliance to bring you MyChart, a shared electronic database and record. Whether you receive your patient care through a Valley Health or an Inova Health System provider, you will have access to your healthcare information through this one, secure patient portal.
How do I Register on the mosaic health portal?
Tell a Mosaic Health staff member you wish to be registered on the portal. The process takes only a few minutes. What happens after I get registered? You will receive an email (from Mosaic Health Community Health Center) at the email address you have given us. The email will have your username and a button to click that says ”Setup Portal Account”.
How do I Activate my MyChart account?
New to MyChart? Call Variety Care at 405-632-6688 and select option 5 to activate your account. Urgent Healthcare Matters Please do not use MyChart to send any messages requiring urgent attention. For medical or behavioral health metters, call the Signature Health office where you are a patient.
What to do if you don’t have refills?
If you have refills remaining, please contact your pharmacy directly. If you do not have any refills and need a new prescription from your provider, you can quickly & easily request them

MyChart is a free service offered to patients of the Prince George’s County Health Department that provides patients secure on-line access to portions of their medical records. Now, no matter where you are, at home, at work, or on the road, you can securely and conveniently use the Internet to help manage and receive information about your health. If you have received health care services at any health department clinic, your medical record will be found in MyChart.

You are able to schedule your own vaccine appointment on MyChart, walk into any VG clinic during regular business hours or attend one of our mobile community vaccination events.
