Important Notice Beginning February 17, 2023, Mosaic’s patient portal will become view-only. This change allows Mosaic care teams the ability to finish anything currently open and to begin moving the information over to Epic, Mosaic’s new electronic medical record (EMR).
How do I set up the mosaic Health Community Health Center portal?
You will receive an email (from Mosaic Health Community Health Center) at the email address you have given us. The email will have your username and a button to click that says ”Setup Portal Account”. What is the website to get to the portal?
Can I set up my mosaic MyChart account if I am a patient?
Yes. You must be a current patient to set up your Mosaic MyChart account. When you text 541-383-3005 for your code, our team will use your phone number or other identifying information to locate your record in our system and activate your account.
