When did Mount Carmel change its electronic health record system?

Mount Carmel changed its electronic health record system in October 2021. MyChart may not have all historical visit information and test results available for visits that occurred before this date.  Our previous patient portals are still accessible but as read-only.

How old do you have to be to have a MyChart account?

Your child (over age 12) may request their own MyChart account. They may also grant complete access to their information.

How to schedule an appointment with Catholic Health?

If you would like to arrange an Catholic Health eVisit appointment with your personal Catholic Health Medical Group primary care physician, please call your physician’s office to schedule. The office will ensure your eVisit is scheduled with your own physician.

Does Catholic Health MyChart have proxy access?

If you currently have Proxy Access to your child’s Catholic Health MyChart account, as of their 12th birthday the access will change to “limited” access. Access to view Allergies, Immunizations, Preventive Care, Health Summary, Medical History and Hospital Admissions will continue.
