Does Mount Carmel have a patient portal?

Our new patient portal is integrated with other member hospitals and providers from across the country. Mount Carmel changed its electronic health record system in October 2021. MyChart may not have all historical visit information and test results available for visits that occurred before this date.

We’ve moved to a new electronic health record and billing system that allows us to better meet your needs. Our new electronic health record, Epic, allows you to access information about the care you receive at any Mount Carmel facility or Mount Carmel Medical Group office in one patient portal called MyChart.

When did Mount Carmel change its electronic health record system?

Mount Carmel changed its electronic health record system in October 2021. MyChart may not have all historical visit information and test results available for visits that occurred before this date.  Our previous patient portals are still accessible but as read-only.

How long does it take to get a MyChart account?

We need some information from you in order to grant you a MyChart account. Once submitted, it could take up to a week for processing and verification. Once approved, you will receive an email or a letter with your activation code and instructions on how to activate your MyChart account. If you have any questions, please contact your health care provider.

How does a third party verify your identity?

A third-party verification system confirms your identity by checking responses that you provide against their database of information. None of this information is stored with your health care provider. Answer questions from a third-party verification system. Once verified, proceed to create your username and password.

How old do you have to be to use MyChart?

MyChart self-signup is available for patients over 18 years of age. If you are looking to establish a proxy relationship for a child or adult family member, please contact your health care provider’s office for assistance.
