What is MyChart and how do I use it?
MyChart offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With MyChart, you can use the Internet to: Request medical appointments. View your health summary from the MyChart electronic health record.
What is MyChart care companion?
MyChart Care Companion This function provides reminders to patients to complete tasks. This will be set up by the provider if it applies to the patient’s care. Happy Together Happy Together allows patients to connect their MyChart account(s) with other organizations to their MVHS MyChart.
What is Duke MyChart?
Duke MyChart, our secure, online website, makes it easy for you to schedule appointments, pay your bills, and send a message to your doctor’s office when you have non-urgent medical needs. To sign up for Duke MyChart, please call us at 919-620-4555 or 800-782-6945 to receive an activation code.

As a Patient Centered Medical Home (PCMH), MVHC provides a team of healthcare workers who will care for all of your health needs. Studies show that PCMH: provides better support and communication; creates stronger relationships with your providers; saves you time. Don’t get confused by the word “home”. A patient-centered medical home is not like a nursing home and your care team is not going to your home. The most important job of a medical home is to take care of you and your family. Not just your high blood pressure or diabetes, but all of you. Your care team gets to know you and your family personally. They care about you while caring for you. PCMH puts you at the center of your care, working with you to create a personalized plan care that navigate you to your optimum level of health and wellness.
When was MVHC founded?
Founded in 2008, MVHC serves as a best-in-class health care resource to ensure all members of the communities we serve have access to high-quality, affordable health care. As your “community of care,” we are dedicated to serving the primary care needs of the residents in each of our convenient locations in Muskingum, Morgan, Coshocton and Guernsey counties.
