How do I access my HMG and Hoag urgent care medical records?
Hoag Medical Group (HMG) is pleased to announce we are now offering you access to your HMG and Hoag Urgent Care medical records through Hoag Connect MyChart. Hoag Connect MyChart is easy to download and gives you access to: View test results. Communicate with your provider.
How much does Hoag charge for medical records?

Is MyChart password-protected?
MyChart is password-protected and delivered via an encrypted connection. To learn more about privacy, visit the Hoag Privacy Policy. PLEASE NOTE: Certain tests require a return visit to get results, including STD testing. If a return visit is required, your provider should have instructed you when to return.

Who is required to obtain medical records?
To obtain medical records copies, a signed authorization is required from the patient or authorized representative*. *Authorized representative includes parent of a minor patient (under 18 years of age), legal guardian, or is named in the patient’s Advance Health Care Directive as agent to make health care decisions.
