How do I access my HMG and Hoag urgent care medical records?
Hoag Medical Group (HMG) is pleased to announce we are now offering you access to your HMG and Hoag Urgent Care medical records through Hoag Connect MyChart. Hoag Connect MyChart is easy to download and gives you access to: View test results. Communicate with your provider.
How much does Hoag charge for medical records?

Is MyChart password-protected?
MyChart is password-protected and delivered via an encrypted connection. To learn more about privacy, visit the Hoag Privacy Policy. PLEASE NOTE: Certain tests require a return visit to get results, including STD testing. If a return visit is required, your provider should have instructed you when to return.

Is Hoag Medical Group redirected?
Please note that several Hoag Medical Group (H MG) physicians are being redirected to assist in the hospital to care for patients during this recent surge in hospital admissions. If you have an appointment scheduled at HMG, you may experience a longer wait time or your appointment may be canceled or rescheduled for this reason. Who is required to obtain medical records?
To obtain medical records copies, a signed authorization is required from the patient or authorized representative*. *Authorized representative includes parent of a minor patient (under 18 years of age), legal guardian, or is named in the patient’s Advance Health Care Directive as agent to make health care decisions.
