What is mymemorialchart?
myMemorialChart offers you personalized and secure online access to portions of your electronic medical record (EMR), where your provider maintains your health information. It enables you to securely use the internet to help manage and receive information about your health wherever and whenever you need.
Can I create a MyChart account for free?
If you have received care at Memorial, you can create a free MyChart account in just a few steps. My healthcare provider has given an activation code to me. I would like to signup to receive an activation code.
How do I give someone else access to my chart?
You can give someone else access to your MyChart. This can be done through the “Share My Record” option under “Sharing.” Once you’re on the “Share My Record” screen, select the “Friends and family access” option. Note: You’ll need to provide the email address of the person you’d like to have proxy access to your chart.
How can MemorialCare now care for me?
• We are available to care for you through in-person primary care or urgent care visits as well as through video visits. Visit MemorialCare Now for more information on how to schedule an appointment.

When will my blood test results be available?
Your test results will be available online after your healthcare provider has reviewed and manually released them.
Where does my memorial chart come from?
Your myMemorialChart information comes directly from your electronic medical record at your provider’s office and our hospitals. You can send a myMemorialChart message asking your provider to correct your medical record or call their office directly. You can also have your provider update the information at your next visit.
