In partnership with OCHIN, who supports our clinic’s electronic health record system, we are offering our Cowlitz Family Health Center patients secure, online access to their personal medical charts. Using the OCHIN MyChart, patients can have online access to their medical information, any time of the day or night.

What is MyChart and how do I get help?

What happens if I enrolled in MyChart before or after my appointment?

MyChart displays information from the day of enrollment, forward. If you enrolled in MyChart before your appointment, your provider should be on your list of provider options in MyChart. If you enrolled in MyChart after your appointment, your provider will be added to your list of care team members after your next appointment.

What to do if you don’t have refills?

If you have refills remaining, please contact your pharmacy directly. If you do not have any refills and need a new prescription from your provider, you can quickly & easily request them
