What is the Patient Portal? The Patient Portal allows patients access to their medical records at EFP through the secure website at www.endwellfamily.com. Patients will have access to their electronic chart including their demographic information, allergies, medications, vital signs, immunizations, procedures, and lab results.

myCHC is a secure way to talk to your provider, request referrals, see upcoming appointments, and see a summary of your visit. You can also download the healow app to use directly on your phone. The Patient Portal is for non-urgent issues only. If you have a true medical emergency, please dial 911.

Patients who register for the AKDHC Patient Portal have access to pertinent health information 24/7 which includes, visit summaries, lab and imaging results, patient education materials and more. Patients can also exchange secure portal messages directly with their physician’s staff.
How do I upload my Patient’s results to the portal?
Once the patient is in their portal account they will see tabs on the top of the screen. Choose the “CHART” tab, click results, and then click view chart as PDF. It will then upload the patient’s results to view.
