How old do you have to be to use MyChart?
I understand that I must be age 18 or older to request a MyChart account. Minor children between the ages of 14 and 17 (inclusive) are not eligible to participate in MyChart services. Parents or guardians of minor children may request proxy access to the minor child’s account by following the terms outlined in the “Proxy Access” section of this Terms of Use and Agreement. Under State and Federal law, there are certain types of medical information that the parent or guardian of a minor patient age 14 or older may not view without the consent of the minor patient. Because of these requirements, we cannot offer the full spectrum of services to parents of minor patients in this age category. Thus, MyChart services for parents of minors between 14 and 17 (inclusive) will be limited to messaging and making future appointments. When the minor patient reaches the age of 18, all proxy access will be automatically disabled. The patient may request enrollment for a personal MyChart account.
Where to send a written notice to HPH?
If I have proxy access to the MyChart account of a patient who is incapacitated and my legal relationship with patient changes, I understand I have a responsibility to inform HPH immediately by sending written notice to the patient’s clinic or the HPH Privacy Officer, Hawaii Pacific Health, 55 Merchant Street, 26th floor, Honolulu, HI 96813. HPH reserves the right to revoke proxy access at any time for any reason.
Is MyChart available to all users?
MyChart may not be available to users all the time. Access is provided on an as-is, as-available basis. HPH does not guarantee users will be able to access MyChart at any particular time due to system failures, back-up procedures, maintenance, or other causes beyond the control of HPH. HPH reserves the right to change the features of MyChart and these Terms of Use and Agreement at any time. The current Terms of Use and Agreement can be viewed at any time by clicking on the Terms of Use and Agreement link on the MyChart website.
