How will I receive copies of my health information?
You will receive the information electronically in an encrypted secure email that you will provide on your application and valida te at request submission.
Can I still do the paper form?
Yes, if you are unable to scan and send picture identification, you can print the paper form, complete it and send it to the appropriate facility. You will need to be sure to legibly complete each field on the form.
Where do I send the completed form if in paper?
On the Health + Hospitals website is a listing of all addresses and contact numbers to call regarding copies of health information from each location of Health + Hospitals.
