Can a surrogate access a patient’s MyChart account?

Note: If a physician determines a patient does not have medical decision-making capacity, an appropriate surrogate ( i.e., Power of Attorney for Health Care, Health Care Representative, Court appointed legal guardian, or other surrogate under Indiana State law) may request and be granted full access to a patient’s MyChart account ; provided, the individual requesting access must provide copies of any legal documents verifying the surrogate’s authority to have access to the patient’s medical information.
