MyChart is not to be used for urgent medical needs. For medical emergencies, dial 911. Patients who have been admitted to Covenant may also set up access to MyChart to view their Covenant medical records. How to appoint someone to access my medical records?

If you would like to appoint someone else to have the ability to access your medical records through the portal please complete the Adult Proxy form. Please submit the form to Medical Records. Keep a copy for your records.

How old do you have to be to sign a medical form?

Does Scheurer Health have a thrive portal?

The Thrive Patient Portal is no longer the main medical record for Scheurer Health patients. If you do not have a MyChart account, in need of medical information that took place before March 14, 2020 and it took place at Scheurer Hospital, click the button below.
