To keep you connected with your care team, our online Patient Portal offers a simple and convenient way to make appointments, make payments, communicate with your doctors, view test results, access visit notes, explore your health history and request prescription refills.
How do I request an email invitation to the patient portal?
You may also contact your Summit Health provider’s office to request an email invitation to the patient portal. A member of our staff will contact you to confirm appointments after requesting. Please remember this portal is not to be used for medical emergencies. In case of medical emergency, please dial 911.
How do I register for the patient portal?
You must register before you can log in to the patient portal. To register, click on the “Create Account” link above, or contact your provider’s office and ask them to send you an email invitation. The email will contain a secure link which will allow you to complete the registration process.
