Spectrum Health MyChart (including both the mobile application and the website and all tools, services, content and information available on or through the mobile applications and the website), is made available through Spectrum Health and all of our parent, subsidiary and related organizations (“we” or “us”).

What is the six-character code you’ll receive when you log in to MyChart?
The first time you log in to MyChart on your computer or after updating the Spectrum Health App, you’ll receive a six-character code via email or text message. Enter this code to access your health information. The code is valid for 30 minutes.
What does Lakeland MyChart include?
Lakeland MyChart only includes medical information gathered from a Spectrum Health Lakeland facility or local medical practices using our electronic health record system. This includes imaging and laboratory test results. So much more!
How long does Spectrum Health MyChart take to be enforced?
Any cause of action you may have with regard to the use of Spectrum Health MyChart must be instituted within six months after the claim or cause of action arises or be forever waived and barred. If any court of competent jurisdiction finds any provision of these Terms of Use to be unenforceable, such provision shall be enforced to the maximum extent permissible, and the remainder of these Terms shall be separately enforced.
What to do if you suspect unauthorized use of Spectrum Health?
Contact Spectrum Health immediately if you suspect unauthorized use of your Account, or any other unauthorized activity on your Account. You must exit fully out of your Account at the end of each session. You are and will be responsible for all activities conducted on Spectrum Health MyChart that make use of your Account, including any use you may subsequently contend was not authorized by you.
What is a submission license?
Submission; License. You may submit personal and/or health information, comments, ideas, suggestions, photos, reviews and other content (the “Content”) through the App or Website. We are not responsible for any Content you provide. Upon submission, you grant us a perpetual license to use, display or otherwise exploit the Content, other than personally identifiable health information, under a perpetual, worldwide, royalty-free, fully paid-up irrevocable right and license, which includes a right to sublicense rights of exploitation. Our rights to the Content will apply to the Content in any form or medium, whether now known or hereafter known.

Our trademarks cannot be used without an express, written license agreement.
