Can I establish my own Spectrum Health MyChart account if I am over 12 years old?
Your parent or legal guardian will have access to your account until you reach 12 years old. If you are over 12 years old, we may give you access to Spectrum Health MyChart to establish your own account, without parental/guardian access. Additional information is available in the FAQ’s.
What medical information is included in MyChart?
Please note that MyChart includes only medical information gathered from a Lakeland Health or Southwestern Medical Clinic physician office using our electronic health record system. This includes imaging and lab test results. View lab and imaging results performed at a Lakeland Health facility.
What is the process for resetting a password for a Spectrum Health App account?
Provide your user name, primary phone number, and date of birth. Check your email address for a code. Enter this code on the log in page. Follow the directions to reset your password. If you get locked out of your account, you’ll need to contact Spectrum Health App Support to reactivate your account.
What happens if you have a claim or cause of action against Spectrum Health MyChart?

REGARDLESS OF THE FORM OF ACTION OR THEORY OF RECOVERY, IN NO EVENT SHALL SPECTRUM HEALTH, ITS AFFILIATES, ITS LICENSORS AND/OR ITS SERVICE PROVIDERS BE LIABLE OR RESPONSIBLE TO YOU IN ASSOCIATION WITH SPECTRUM HEALTH MYCHART, OR YOUR USE THEREOF, FOR ANY: (A) INDIRECT, CONSEQUENTIAL, INCIDENTAL, SPECIAL, PUNITIVE OR EXEMPLARY DAMAGES, EVEN IF THEY ARE AWARE OF THE POSSIBILITY OF SUCH DAMAGES; OR (B) LOST PROFITS, LOST REVENUE, LOSS OF DATA, LOST EXPECTANCY OR BUSINESS INTERRUPTIONS. ANY CLAIM RELATED TO SPECTRUM HEALTH MYCHART MUST BE INITIATED WITHIN ONE (1) YEAR OF THE DATE OF THE EVENT THAT GAVE RISE TO THE CLAIM.
