How do I access MyChart?
For your security, photo identification is required. UW Health patients can access MyChart on their iPhone and Android devices through a free application. You must set up a MyChart account before using the app on your smartphone. iOS: From your device, go to the Apple Store and search “MyChart.”
Who is eligible for MyChart?
Minors age 12-17 may be eligible for their own MyChart account with permission from a parent or guardian. At least one parent or guardian must set up and maintain proxy access to the minor child’s account and a distinct email address must be provided for the minor. DEACTIVATION:Participation in MyChart is voluntary.
Can I access my family member’s medical record through MyChart?
You can use MyChart to access the medical record of another family member (such as a child, parent, spouse or partner), or a close friend, through a MyChart feature called “proxy.” Proxy access may also be granted to others from within one’s own MyChart account.
