Baylor Scott & White – Facilities (BQA and Dallas Endo Providers Only) Blue Cross Blue Shield Medicare Advantage HMO (DFW & Houston Markets Only) TDDC will work with your health plan representatives and your primary care physician to obtain prior authorization and, if necessary, referral for your gastroenterology procedure (s).
