The myMemorialCare myChart electronic medical record, powered by Epic’s myChart is your personalized Internet connection to your doctor’s office. You can schedule appointments, request prescription refills, review your health history and more—online, any time.
Does Torrance Memorial have a patient portal?
The patient portal is available to all Torrance Memorial patients who are 12 years of age and older. Are there any fees to use the portal? No, it is a complimentary tool provided to you by Torrance Memorial. There are no charges for using the portal. Family and Proxy Access Can I request access to my child or another adult’s portal information?

What if I can’t self-enroll in Torrance Memorial?
If you’re unable to self-enroll after submitting your SSN# in the self-enrollment form, please contact Health Information Management at (310) 517-4721. If you are a Torrance Memorial patient and are at least 18 years of age, you will be enrolled in MyTorranceMemorial when you provide your email address to a registration team member.

Log into myChart and from the left menu, go to the Preferences section and select the appropriate option.
How to reset my chart password?

How long does it take to receive a response from MyChart?
You will generally receive an answer within 1-3 business days. Please note that myChart should not be used for urgent situations. Please contact your medical center if the situation requires immediate attention or dial 911 if it is an emergency.
How long does it take for my chart results to be released?
Your test results are released to your myChart account after your physician has reviewed them. This is generally within 1-12 days.
Where does my chart come from?
Your myChart information comes directly from your electronic medical record at your doctor’s office. Ask your doctor to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.
