MyChart offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With MyChart, you can use the Internet to: Request medical appointments. View your health summary from the MyChart electronic health record.
What is Duke MyChart?
Duke MyChart, our secure, online website, makes it easy for you to schedule appointments, pay your bills, and send a message to your doctor’s office when you have non-urgent medical needs. To sign up for Duke MyChart, please call us at 919-620-4555 or 800-782-6945 to receive an activation code.
How to access my child’s chart?
For access to a child’s chart fill out the “ Request Proxy Access to My Child’s Account ” form on our MyChart home page. You can also ask about this at your child’s next visit.
What is MyChart activation code?
Patients who want to take part will get a MyChart activation code during their clinic visit. The code will allow you to log in and create your own username and password.
How long does MyChart activation code last?
Once your information has been verified, you will receive an email within 7 business days with your activation code and instructions on how to log in to your MyChart account. What to do if my chart is not correct?
If you see any health information on MyChart that is not correct contact your doctor. Your MyChart information comes directly from your electronic medical record at your doctor’s office. Ask your doctor to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.
Where is the activation code for MyChart?
To sign-up for access, visit the MyChart patient portal. You’ll need the activation code located on your billing statement or your after-visit summary. This code will enable you to log in and create your own username and password.
