How many people can access MyChart?
Only one person will be granted proxy access to a MyChart account. A MyChart account will be activated for both the proxy and the account holder. If the proxy’s legal relationship with the account holder changes, the proxy must inform United Regional immediately by sending written notice to: Medical Records.
How to request proxy access to MyChart?
Individuals may request proxy access to another individual’s MyChart account by completing the Proxy Access portion of the MyChart Consent Form. Such access will only be granted to parties with parental rights or legal guardianship over the MyChart account holder and only to the extent that the party requesting proxy access can demonstrate the legal right to account holder’s medical information, or if the patient provides written authorization. How old do you have to be to get a MyChart account?
Unless otherwise allowed by law, an individual must be age 18 or older to request a MyChart account. If allowed by law, Parents or guardians of minor children may request access to the minor child’s account by following the terms outlined in the “Proxy Access” section.

By accessing or using MyChart, you agree that you have read, understand, and agree to be bound by these Terms and Conditions, as amended from time to time. Your continued use of the service after the date of amended Terms and Conditions will constitute your acceptance. If you do not agree to the Terms and Conditions when amended, you may stop using the service and cancel your account. The terms “United Regional,” “we,” and “our” refer to United Regional Health Care System, including the hospital, and United Regional Physician Group.

Access to MyChart is a courtesy and not a patient right. United Regional reserves the right in its discretion to discontinue your access. The following is a list of reasons which may result in deactivation. The list is not inclusive and each circumstance will be evaluated individually.
