MyChart became available to patients of most CVMC ambulatory departments and outpatient practices on November 9, 2019. MyChart is NOT replacing CVMC’s inpatient health portal—Meditech Central to Your Well Being—at this time.

How do I Activate my MyChart account?

Your username and password will work in MyChart. Fill out the secure account form and click Submit request. Upon submission, you will be sent an activation link by email. Clicking this link from your email returns you to MyChart and inputs the code automatically. Confirm your birthday and click Next.

As of November 9, 2019, you should stop using your existing MyHealth patient portal to access information from outpatient practices and clinics. You will still have access to your information in this portal after November 9, 2019, but no new outpatient information will be added. To view new outpatient medical information and test results, see upcoming appointments, and send messages, you should create MyChart accounts at MyChart.UVMHealth.org.
