How do I access my UR Medicine MyChart?
The app includes access to MyChart, which provides you with secure access to frequently used features of UR Medicine MyChart such as messaging your doctors, upcoming and past appointments, test results and much more. You can easily access your MyChart without needing a computer; download the MyURMedicine app by clicking below.

You can easily access your MyChart without needing a computer; download the MyURMedicine app by clicking below. Please note, not all UR Medicine MyChart functionality is currently available on the mobile app version. *On an iPad, the MyURMedicine App is downloadable through the App used for iTunes.
What can you do with MyChart?
Using MyChart, you can: Easily view your outpatient diagnoses, medications, immunizations, allergies and most lab results. Request prescription renewals and appointments online. Securely and confidentially communicate with your health care team. Electronically request corrections and/or additions to portions of your chart.
What is the Apple Health MyChart connection?
For patients who already use Apple Health to track their physical activity, nutrition, mindfulness or sleep, this connection will let you to see your MyChart data without opening up an additional app. This connection is not automatic and data will only be shared if you give the Apple Health app permission to connect to MyURMedicine:
How to get activation code for URMC?
You also can request an activation code online by visiting mychart.urmc.rochester.edu, or by contacting our MyChart Customer Service Center 8 a.m. to 5 p.m. weekdays: 585-275-URMC (8762), 1-888-661-6162. Patients under 18 (and/or their parent/legal guardian) must speak with their doctor in person before signing up for MyChart.
How to sign up for MyChart?
To sign up for MyChart, you will need an activation code. This code allows you to log on and create your new MyChart user ID and password, which you will then use to log on to MyChart. All patients over age 18 will receive an activation code at the end of each UR Medicine visit, so you can sign up at home when it’s convenient for you.
