How do I access my E-chart?
Fill out the form and fax to 713-383-3702. When UTHS Staff receive the faxed form, you will receive notification in the email account on file of a pending message in My e-Chart. The email notification will have a link to access My e-Chart. Read your log in instructions carefully.
How long does a UtHealth activation code last?
