If you have an activation code from your provider’s offi­ce, a recent bill, or a printed After Visit Summary, visit UVMHealth.org/MyChartCode and enter it. Skip to step 4. You do not need to create a new MyChart account if you had a UVM Medical Center MyHealth Online account.
