Who is eligible for MyChart?

Minors age 12-17 may be eligible for their own MyChart account with permission from a parent or guardian. At least one parent or guardian must set up and maintain proxy access to the minor child’s account and a distinct email address must be provided for the minor. DEACTIVATION:Participation in MyChart is voluntary.

How do I access my teen’s MyChart?

Log into your MyChart. Switch to the proxy account for your 12- to 17-year-old. In the search menu field type in “Teen” and select “Teen Access.” Read the information in the section, ‘MyChart Access for your Teen” and follow the instructions.
