How to request proxy access?

To request proxy access, you will need to either log in to or create your own . MyChart account. After your account has been established, you can find the Request Proxy Access Form under the “My Medical Record” tab. For minors, proxy access will terminate when the patient turns 13 years old.

How long does a proxy code expire?

If you do not have one, you can request an activation code online. Please note that the activation code will expire after two weeks. Proxy access is available for parents and caregivers.

Can WakeMed MyChart be used for urgent needs?
