What is MyChart?
MyChart is an online service offered by Yale New Haven Health that allows you to connect with your doctor’s office, schedule appointments, request prescription renewals, review your health history and more — any time! All you need for access is an activation code from your doctor and your date of birth.
What are Yale Medicine and Yale New Haven health’s billing hours?
We are available Monday through Friday from 8:30 am to 5:00 pm. To deliver a more secure, convenient, and eco-friendly way to view your bills, Yale Medicine and Yale New Haven Health will switch to paperless billing for MyChart users starting January 3, 2023.

What is a mychart?
MyChart is designed to put you, the patient, at the center of your healthcare. You can use MyChart to make appointments, communicate with your healthcare provider and see test results and notes from your visit. If you already have a MyChart account, you can log in with your username and password below.
How to sign up for MyChart?
To sign up for MyChart you need an activation code, which you can get in one of three ways: Find one on your after-visit summary. Request one at your next appointment. Request one online. If you already have an activation code , sign up now.
When can I see a note from my visit?
You will be able to find your notes in MyChart when your provider completes it.
What if I don’t understand something written in the note?
Keep in mind that your provider uses the note to manage your care and to communicate with others involved in your care. It is not necessarily written in lay-person’s language. If you find the note difficult to understand because there are unfamiliar terms or abbreviations, this Medical Dictionary – Common Abbreviations may help. Medline Plus is a good resource to help you better understand your condition or medical terms.
Is it possible for me to see my entire medical record?
All patients have a right to request their full medical record, including providers’ notes from the Yale Health Department of Health Information Management.
