MyChart is an online service offered by Yale New Haven Health that allows you to connect with your doctor’s office, schedule appointments, request prescription renewals, review your health history and more — any time! All you need for access is an activation code from your doctor and your date of birth.

How can I get an activation code for MyChart?

Go to the MyChart Request an Activation Code page to get your activation code. Or you can ask the receptionist at your next visit to email you a code or go to the Member Services desk and ask to be emailed a code. Or after your visit you will receive an After Visit Summary.

Who can I message in MyChart?

When you activate your MyChart account, you will immediately be able to send secure messages to your primary care clinician in Internal Medicine, Student Health, and Obstetrics & Gynecology. If you have proxy access for your child, you will be able to message their primary care clinician in Pediatrics. Please note proxy access for children is not available for children aged 13-17.

When can I see my test results in MyChart?

Results of common lab tests are immediately available in MyChart. Other tests are released 3 or 7 days after finalized , including imaging tests and pathology. Many providers will manually release them earlier, often with explanatory comments.

Are hospital admissions available in MyChart?

If you are admitted to a hospital in the Yale New Haven Health system, you will be able to view your schedule of medications and testing that take will take place throughout your admission. Once you are discharged, you will be able to view your date of admission and discharge along with your discharge instructions.

What is adult proxy?

Adult proxy access for MyChart allows you to securely communicate on behalf of a patient who is 18 years of age or older. In order to obtain access, both the patient and proxy must complete and sign the Access Authorization with Adult Proxy form. The proxy’s access is terminated when the patient makes a written or online request to terminate access, an expiration date specified by the patient is reached, or the patient revokes access from within their MyChart account.
